IN THE CIRCUIT COURT OF FLORIDA, SEVENTH JUDICIAL CIRCUIT
IN AND FOR FLAGLER, PUTNAM, ST. JOHNS AND VOLUSIA COUNTIES


RE: INITIAL GUARDIANSHIP PLAN PHYSICIAN’S REPORT
REF: PB-2005-003-SC (Rescinds PB-2000-003-SC)

WHEREAS, § 744.363 (1)(e), Florida Statutes, requires physical and mental examinations necessary to determine the ward’s medical and mental health treatment needs be included in initial guardianship plans, and

WHEREAS, the judges of the Seventh Judicial Circuit Court have determined that a physician’s report should accompany the initial guardianship plan for purposes of determining the physical/medical status of the prospective ward;

NOW, THEREFORE, I, WILLIAM A. PARSONS, Chief Judge of the Seventh Judicial Circuit of Florida, hereby order that except in cases wherein guardianship of the property of a minor is sought, all initial guardianship plans filed in the Seventh Judicial Circuit shall be accompanied by a physician’s report to aid the court in determining the physical/medical status of the prospective ward.

IT IS FURTHER ORDERED that Administrative Order # PB-2000-003-SC is hereby rescinded.

Nothing in this Order shall prohibit judges from waiving the requirements of this Order in individual cases.

TO BE RECORDED in Flagler, Putnam, St. Johns and Volusia Counties.
